NHS England have shared recent statistics that the majority of adults in England (63%) are living with excess weight. They have requested help from practices to provide support and advice to patients as living with excess weight increases the risk of serious illness or diseases.
Guardian Medical Centre is committed to supporting our patients to live healthier for longer and have recently launched a new appointment type with Joanne our Healthcare Assistant. The appointment is to support our patients with maintaining or achieving a healthy weight, making healthy lifestyle choices and to offer motivational support to encourage exercise. The appointment also includes some general observation checks such as routine blood tests (if you had not recently had any) and a blood pressure reading.
If you have received an invitation for this appointment, your medical records indicate that your most recent BMI (body mass index) is equal to or over 30, and may therefore benefit from some support with your weight and/or lifestyle. We would reassure patients that this is an offer of support for patients who wish to accept it, and there is no requirement to engage with the service.
If you believe your medical records are incorrect or would like to update us on your most recent weight measurement, please submit an anima admin request: https://patients.animahealth.com/signIn
Due to an already busy telephone system, we would politely request that patients do not contact us by telephone to update us or to query this text message received and submit an online response instead.
