New Patient Registration
We welcome and register any person living within the practice area.
Your registration is with the practice and you are able to request to see any of the doctors at the practice.
Registration forms can be obtained from the reception desk.
When you attend the Surgery to register we require the following:
- 1 x proof of identification such as passport, driving license, identity card, bank card, train/bus pass
- 1 x proof of address such as utility bill, bank statement, housing letter
The registration process can take approximately two weeks, one of the clinicians may invite you to come in for a check-up if necessary, these patients will be contacted by letter.
If you have not received a letter within a fortnight you can assume all is well and that you are registered with the practice.
If you have consented to SMS messaging you will receive a text message to confirm when your registration is complete.
